Family Health Network has brought together a solution that makes it easier for families to share and to participate together in the care of each other. Family Health Network connects individuals needing support at home with their extended family members and health care providers via the Internet using intuitive and interactive software programs. These programs capture and deliver important information about a person’s health, social and emotional status on a daily basis. This information can then be used by the family to identify potential needs and issues and provide support or intervention before they become a larger problem. Responses can also be tracked over periods of time to show patterns of a person’s health and general well-being that may be important to share with a physician or other health care provider.
